What Is Health Access Solutions?
HealthPass is a trusted affiliate of Health Access Solutions, who operate the Employee Assistance Program, the Medical Cooperative, and the Dental Cooperative.
Their incredible team handles preventative reimbursements, as well as processing needs for members of Cooperative Medical and Dental.

By Text:
Text "3" to (239) 299-0333 from the primary member's phone number.
If your phone number is registered with us, then any of these messages will automatically send you back a link to login to the member dashboard.
That link will already contain your Member ID and Last Name, and all you'll have to fill in to confirm your identity is the primary member's date of birth.
Complete the short form, along with an invoice, bill, or receipt for the preventative service. (This can include a photo taken from your phone.)
To make sure your reimbursement request is processed without delay, please have the following ready:
Patient Name – Which person on your membership the service was for.
Date of Service – When the preventive care was received.
Eligible Expense Category – For example, dental cleaning, physical exam, or mental health counseling. You can find this information on your member dashboard.
Itemized Bill from the Provider – This must show the services you received and the charges for each.
After you submit your reimbursement request, you’ll receive a confirmation from Health Access Solutions. Once our team has reviewed your request, it will be processed within a few business days. If our team needs any additional information about your request, they will be in touch with you directly.

You’ll need the following information about the Primary Member:
Primary Member ID
Primary Last Name
Primary Date of Birth
(If you do not use your Member ID, use the Member Lookup Tool.)

By Reimbursement:
Up to three wellness checks per member per year with any provider.
Go to the Member Dashboard to submit your itemized bill for reimbursement.
Direct Primary Care:
HealthPass will facilitate a membership with a Direct Primary Care provider through our local and national networks.
Go here to activate this benefit and select your Direct Primary Care provider.
To access the Health Access Cooperative, simply go to your Member Dashboard.
For a Medical event, click "Medical Cooperative".
For a Dental event (preventative and/or non-preventative), click "Dental Cooperative".
All gym memberships for members 13 and older (including with Courthouse Club Fitness in Salem and Superior Athletic Club in Medford) are activated through the Active&Fit network.
Visit Active&Fit to confirm your eligibility, activate this benefit and access your FitnessID.
ScriptCo:
ScriptCo delivers generic medications at wholesale costs.
To activate your ScriptCo membership.
Check your email for a message from ScriptCo for your sponsor number
Go to scriptco.com, click "Join" and enter your sponsor number
Create your account and use it to pay for prescriptions and track your order
To order a prescription through ScriptCo:
Have prescribers send in new prescriptions to ScriptCo Pharmacy by either: E-Scribe (fastest way) or Fax 254-424-9800
ScriptCo will notify you when your prescriptions are ready for payment
Your prescriptions will be sent to your address
Prescription Card
To get the best price on prescriptions at your local pharmacy, access your pharmacy card.
Go to the Member Dashboard
